About The Position

This is an internal 13-week assignment position for a Case Manager at $55 per hour. The Case Manager is responsible for the interdisciplinary coordination of care for a designated patient population. This role involves reviewing inpatient admission records to ensure proper utilization of hospital resources and determination of the appropriate level of care. The Case Manager will assess and identify discharge needs, coordinate appropriate discharge plans, and work collaboratively with the interdisciplinary team to achieve desired financial and quality outcomes. Additionally, the Case Manager will maintain and enhance payor relationships.

Requirements

Licensure in the state where the hospital resides is preferred.
Professional disciplines may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or (licensed) social worker.
Current BLS certification required.
Three (3) to five (5) years of inpatient experience, preferably in an acute, IRF, or LTACH setting.
