About The Position

Valley Health Partners is seeking a Case Manager for their Veteran's Health Program in Allentown, PA. This role involves completing comprehensive assessments, developing care plans, and assisting clients and their families in accessing benefits and community resources. The Case Manager will coordinate care, make referrals, and ensure timely documentation within the Electronic Health Record.

Requirements

  • Two years of experience in clinical/ambulatory care setting.
  • Bachelor's degree in Social Work.
  • Ability to work efficiently with people from a broad range of backgrounds.
  • Proficient computer skills and ability to use the Electronic Health Record to maintain timely, accurate documentation and effective communication with clients, team members, and external partners.
  • Ability to adhere to the National Association of Social Workers Code of Ethics.
  • Knowledge of patient rights and laws relative to those rights, such as HIPAA.
  • Knowledge of psychological principles and theories.
  • Must possess a high degree of emotional intelligence, and empathy towards others.
  • Skill in using logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions, or approaches to problems.
  • Excellent verbal and written communication skills.

Nice To Haves

  • Master's Degree in Social Work
  • LSW, LCSW, or LPC preferred

Responsibilities

  • Completes intakes and assesses the psychosocial and socioeconomic needs and barriers of care for patients/clients/families.
  • Participates as an integrated member of the client’s care team.
  • Develops individualized care plans based on assessed needs.
  • Ensures accurate and timely documentation of all client interactions in the Electronic Health Record for effective communication with clients, team members, and providers.
  • Efficiently manages referrals from internal and external sources, ensuring timely coordination and continuity of care.
  • Coordinates with community partners to ensure access to care and services.
  • Provides education on available resources and empowers patients/clients/families to achieve optimal levels of wellness and independence.
  • Participates in community outreach and engagement events.
  • Assists leadership with process improvement and program evaluation.
  • Prepares and delivers outreach presentations, coordinates events, and promotes awareness of VHP programs/services.
  • Performs other duties as assigned.
