About The Position

Baylor Scott & White Institute for Rehabilitation- Fort Worth is seeking a Case Manager to join our interdisciplinary team. This full-time position will be a blended position supporting our Day Neuro clinic in addition to inpatient rehabilitation populations. The schedule for this position will be weekdays 8:00AM to 5:00PM with no weekend or evening hours. At our company, we support your career growth and personal well-being.

Requirements

• Current Licensure per state guidelines in clinical or related discipline OR a Bachelor's or Master's in a health or human services discipline.

Nice To Haves

• Previous experience in Case Management and Discharge Planning preferred.
• CCM Certification Preferred.

Responsibilities

• Clinical Interventions/Discharge Planning
• Post-acute care planning management
• Payer/Referral Management
• Patient and family education and provision of community resources
