About The Position

In this position, Case Managers provide supportive counseling and care coordination to clients on our New Journeys team, a youth-focused program whose goal is to minimize the trauma, disruption, and potential disability that can result from psychosis and schizophrenia-related symptoms. The New Journeys program works to identify as early as possible individuals who are experiencing psychosis, stabilize their symptoms, strengthen their family & social supports, and help develop the skills and knowledge to manage their condition for the long term. This program serves young people age 15-25 who live in Yakima County and have experienced their first episode of psychosis within the previous year. Case Managers on the New Journeys team support clients with connecting to community resources, building skills for self-advocacy and social participation, and keeping engaged with treatment. In this role, strong communication skills and the ability to build trust quickly are essential. This position will work Monday-Friday, 8am-5pm and rotating on-call.

Requirements

  • Bachelor's degree in Behavioral Science or related field
  • Must be eligible for registration as Agency Affiliated Counselor
  • Must have valid driver's license and vehicle liability insurance

Responsibilities

  • collaborating with each client to create an individualized treatment plan
  • assisting clients with obtaining benefits and community services
  • serving as protective payee for assigned clients and assisting with budgeting and tracking spending
  • visiting a client in their home or apartment to deliver medications, check on safety issues, or assist with life skill development
  • coordinating with other service providers and treatment teams to expedite services and advocate on the client’s behalf
  • monitoring clients’ progress, symptoms, and medication response
  • facilitating educational, behavioral health, or skill-building groups
  • completing all documentation in a respectful, client-centered manner
