About The Position

AMG Specialty Hospital - Central Indiana Muncie Campus is seeking a full-time Case Manager (RN or LPN). The role makes special services readily available to patients, their families, and/or significant others to facilitate adjustment and continuity of care after discharge. The Case Manager oversees coordination of admissions and discharge planning, case communications, conferencing, insurance relations and assurance through managed care systems, and verbal and written reporting. The role also ensures that regulations regarding patient rights are fulfilled. The hospital is a Long-Term Acute Care hospital specializing in complex medical needs. Its multidisciplinary approach includes clinical and therapeutic interventions and family involvement, with a high staff-to-patient ratio intended to support individualized attention.

Requirements

Current Indiana Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in good standing.
Current BLS certification.
Current ALS certification.
Minimum of 1–3 years of related work experience as a Case Manager.
Ability to read and communicate effectively in English.
