About The Position

Responsible for managing service plans for PSS clients and coordination with Adult Protective Services and Personal Support Assistants (PSAs). Home visitation of PSS clients, semi-annual assessments and attendance at all required meetings. Assist with orientation and in-service. Essential office function: maintain case files; word processing, data entry, forms and presentations development, mailing, and scheduling.

Requirements

  • Graduation from an accredited four year college or university including or supplemented by twenty one (21) quarter hours or fourteen (14) semester hours in behavioral sciences (i.e. social work, psychology, sociology, and/or home economics).
  • Must have good communication and organizational skills and be able to work independently.
  • Proficiency in Word, Excel and PowerPoint.
  • Must have reliable transportation and be able use personal vehicle for travel in eight county region.
  • Ability to work independently and maintain regular, cyclical schedule in order to meet required deadlines.
  • Must be organized and detail oriented.
  • Must have acceptable results from all required background screenings.

Nice To Haves

  • At least two years experience working in social services field preferred.
  • Experience working with elderly and disabled preferred.

Responsibilities

  • Intake, processing, and tracking of all referrals made by APS or other source. Assign/reassign PSAs to clients based on needs and skills.
  • Scheduling, attendance, and reports of monthly meetings with APS Counselors and PSAs to discuss client service plans and coordinate PSS services.
  • In-home reassessment of all client cases not done by APS at least every 6 months. Visitation of clients in the home as necessary and completion of documents including surveys, applications, assessments, and termination forms.
  • Assistance in processing of PSA documentation and outcome measurement surveys.
  • Assistance in monitoring PSS operations including execution of Plan of Care, efficiency of service plans, training needs of PSS staff, and special projects as assigned.
  • Community awareness and involvement including professional representation of PSS and maintaining close working relationships with other community agencies and resources.
  • Attendance of all required meetings and in-services.
  • Abide by policies and procedures as outlined in the FTHRA Personnel Policy Manual, SSBG Program Standards Manual, and Department Policy Manual.
  • General office duties includes: maintaining case files; word processing, data entry, forms and presentations development, scheduling, faxing, filing, answering the phone, photocopying, and preparing mailings for Personal Support Services Programs.
  • Adhere to work schedule provided.
