About The Position
The Case Manager coordinates health care decisions using a systematic approach to improve treatment quality and outcomes, coordinate care across the continuum, promote cost-effective care within the allotted time frame, ensure hospital-based services meet patient-related utilization management criteria, and implement safe and appropriate discharge plans. The role also assesses patients’ psychosocial needs and provides intervention as part of discharge planning. The primary job functions include clinical interventions and discharge planning, care planning management, fiscal management, and payer/referral management. The position is per diem at Select Rehabilitation Hospital of Denton in Denton, Texas.
Requirements
- Current licensure per state guidelines in a clinical or related discipline, or a bachelor's or master's degree in a health or human services discipline.
Nice To Haves
- Previous experience in case management and discharge planning.
- CCM certification.
Responsibilities
