About The Position

The Home Care Case Manager is the central point of coordination for clients and caregivers within assigned areas. This role is primarily responsible for building and maintaining caregiver schedules, ensuring daily coverage, monitoring EVV compliance, and supporting caregivers so that services are delivered safely, reliably, and on time.

Requirements

  • High School Diploma or GED required, bachelor’s degree preferred.
  • Minimum 2+ years in home care, healthcare, or scheduling.
  • Proficiency in Microsoft Excel, Word, and Outlook.
  • Excellent communication and organizational skills.
  • Ability to work full-time in-office with occasional weekend on-call.

  • Experience with HHA eXchange preferred.

Responsibilities

  • Manage and maintain daily caregiver schedules for assigned clients.
  • Match caregivers to clients based on skills, availability, location, and personality fit.
  • Identify open shifts and fill staffing needs quickly.
  • Monitor caregiver attendance and ensure services are not missed.
  • Coordinate last-minute call-offs and schedule changes
  • Monitor caregiver clock-in and clock-out activity through EVV (HHA eXchange).
  • Follow up on missed punches and EVV issues.
  • Track caregiver compliance documents such as TB tests and background checks.
  • Ensure scheduled hours align with authorized hours.
  • Serve as a primary point of contact for clients regarding schedules and service concerns.
  • Conduct bi-weekly client phone check-ins.
  • Escalate concerns to supervisors when needed.
  • Build strong relationships with caregivers and provide performance support.
  • Monitor KPIs: EVV compliance, missed visits, billed hours, retention.
  • Address caregiver attendance and performance issues.
  • Issue write-ups when necessary, with supervisor guidance.
  • Document communications, schedule changes, and incidents.
  • Work closely with the scheduling team, HR, and management.
  • Communicate with payer sources as needed.
  • Support scheduling and service improvement initiatives.
