About The Position

Merakey is seeking a Case Manager to join their Behavioral Health Urgent Care (BHUCC) team in Philadelphia, Pa. This is a full-time, first shift position, Monday - Friday, 10:30 am - 7:00 pm EST, with an hourly rate of $26.34. The Behavioral Health Urgent Care/Walk-In Center serves adults needing non-crisis, same-day care for emotional or behavioral health concerns. The goal is to provide resolution-focused services and aftercare planning to prepare individuals for long-term recovery and connection to ongoing care, regardless of income or insurance status. The service aims to reduce avoidable visits to crisis centers and emergency rooms by connecting individuals with needed mental health care promptly.

Requirements

Bachelor’s degree in Social Work, Psychology, Counseling, or a related field.
Previous experience in case management, social work, or related roles, preferably in behavioral health or crisis settings.
Knowledge of public and private benefit programs and community resources related to Social Determinants of Health (SDOH).
Excellent communication and interpersonal skills to engage effectively with individuals and colleagues.
Empathy, compassion, and a person-centered approach.
Ability to work collaboratively in a multidisciplinary team environment.
Strong organizational and time management skills to manage case management responsibilities effectively.
Familiarity with electronic health records (EHR) systems and healthcare technology.

Responsibilities

Provide immediate and follow-up case management services to individuals experiencing urgent behavioral health challenges.
Offer comprehensive support, including benefit enrollment.
Provide information about external resources.
Address the Social Determinants of Health.
Make referrals to appropriate services.
Complete outreach contact calls.
