The Contra Costa Adult Continuum of Services (CCACS) is a program of Contra Costa Health Services Health, Housing and Homeless Services Division, in partnership with Heluna Health. Our mission is to provide safe, interim housing with comprehensive services that assist homeless adults in securing permanent housing that will end their homelessness. We respect human dignity and believe housing is a right that is supported through advocacy and collaboration. This role involves establishing and maintaining a caseload, conducting needs assessments, assisting clients with obtaining identification and benefits, and coordinating access to various resources such as housing specialists, mental health services, and legal aid. The Case Manager will also educate clients on daily living skills, facilitate relationship-building, and report client progress in team meetings. A specific focus for the CCACS Case Manager (Income) is to assist clients in obtaining or increasing income through applications for public assistance or employment, conducting outreach to employment resources, and educating clients on budgeting and financial management.
