Work with individuals and facilitates groups to educate clients and promote optimum mental health, well-being, abstinence from substance use, and relapse prevention. Assists individuals in coping with addictions, substance use, and related issues such as family, parenting, marital problems, suicide, stress management, problems with self-esteem, aging, mental health, and emotional health. Conduct intake, screening, assessment, treatment planning, discharge and referral of clients as appropriate. Act as client advocate to coordinate required services or to resolve emergency problems in crisis situations. Complete required clinical documentation according to agency standards including individual and group progress notes; and all other tasks assigned by supervisor.
