About The Position

CAMBA is a community organization that offers over 180 integrated programs in various areas including Education & Youth Development, Family Support, Job Training & Employment Support Services, Health, Housing, and Legal Services. CAMBA operates shelters for single adults and families, providing comprehensive services to help individuals transition into permanent housing. The Kensington Family Shelter, funded by the New York City Department of Homeless Services, offers a supportive environment for 64 homeless families, aiming to move them towards self-sufficiency and permanent housing. The facility includes common areas and on-site security and social services.

Requirements

  • Bachelor's degree (e.g., B.A., B.S.W.) and/or equivalent experience.
  • Must obtain State Central Registry (SCR) clearance.
  • Must obtain The Register of Substantiated Category One Cases of Abuse or Neglect aka Staff Exclusion List (SEL) clearance.
  • Must obtain Criminal history information checks via fingerprinting with New York State Division of Criminal Justice Services prior to start of employment.
  • Ability to maintain clearances throughout the duration of employment.

  • May be required to become First Aid/CPR certified.
  • May be required to become certified in overdose prevention.

Responsibilities

  • Establish and maintain professional relationships with clients and client confidentiality to engage them in case management services.
  • Review documentation to establish client eligibility for the program and make file copies.
  • Create and maintain client files.
  • Input client data and client progress information into the CARES database.
  • Conduct initial intake and psycho-social assessments of clients and their families' needs, and perform periodic re-assessments.
  • Collaborate with clients to prepare initial and periodic revisions of independent living plans, including short-term and long-term client goals.
  • Assist clients in achieving their goals by identifying and locating community resources.
  • Refer clients to permanent housing opportunities and appropriate services within and outside CAMBA.
  • Schedule appointments for clients with referral organizations.
  • Escort clients to appointments (e.g., housing, entitlements, educational, medical, social services).
  • Assist clients in completing applications for benefits and entitlements, or process applications on their behalf.
  • Work with clients to overcome barriers to their goals and assist them in advocating for themselves towards self-sufficiency.
  • Recommend and implement strategies to encourage clients' full participation in the process.
  • Monitor clients' progress toward their goals through regularly scheduled telephone contact and/or face-to-face home and office visits, documenting via progress notes.
  • Follow up with clients and referral organizations regarding client contact and progress.
  • Act as a client liaison/advocate with outside organizations on matters such as education, healthcare, housing, and legal issues.
  • Provide all required information for weekly, monthly, quarterly, and annual reports.
  • May recommend closing of cases for clients who have achieved primary goals and maintained stability, have not participated in the process, or have become ineligible for services.
  • May follow up with clients for a period after successful completion of their primary goals to ensure stability.
  • May complete Relocation Case Review (RCS) for referral for aftercare services.
  • May prepare marketing materials for the program.
  • May prescreen clients over the telephone for eligibility and schedule intake appointments.
