About The Position

SilverStay is a company focused on revolutionizing hospital discharge processes. They aim to improve collaboration between hospitals, families, and post-acute providers to ensure smooth, quick, and compassionate patient discharges. Their model combines embedded hospital support with advanced data tools to reduce Length of Stay (LOS), address weekend discharge challenges, and transform care transitions. They are expanding their services to cover weekends, ensuring patients can be discharged any day of the week and helping hospitals achieve their LOS targets.

Requirements

Licensed RN, LMSW, or LCSW (required)
At least one year of on-site hospital adult in-patient case management and discharge planning experience (required)
Knowledgeable with EMR/EHR & Referral Software
Strong communicator with empathy, composure, and follow-through
Confident navigating post-acute care (SNF, LTACH, ALF, Rehab, & Home Health)
Able to lead independently in a dynamic hospital environment
Energized by impact — you like to see tangible results from your work
Available to work up to 10 hours per shift (Saturday and/or Sunday)
Available to attend initial training onsite 4 consecutive days (Monday - Thursday, 8:30am-5:00pm) and 5 days of job shadowing (non-consecutive) before solo shifts begin. All training time is paid time.

Responsibilities

Facilitate safe, efficient weekend discharges.
Collaborate with hospital staff, physicians, and families to maintain communication and discharge flow.
Utilize SilverStay's Complex Case Tool (CCT) for real-time insights, discharge tracking, and accountability.
Secure weekend admissions by engaging with SNFs, ALFs, rehab, and home health providers.
Act as the hospital's weekend discharge leader, reinforcing accountability, providing guidance, and modeling best practices.
Document weekend progress, barriers, and outcomes, and share updates with hospital and SilverStay leadership.
