About The Position

This is a 13-week assignment position for a Case Manager. The role involves the interdisciplinary coordination of care for a designated patient population, reviewing inpatient admission records to ensure proper utilization of hospital resources and determining the appropriate level of care. The Case Manager will assess and identify discharge needs, coordinate appropriate discharge plans, and work collaboratively with the interdisciplinary team to achieve desired financial and quality outcomes. Maintaining and enhancing payor relationships is also a key responsibility.

Requirements

Licensure in the state where the hospital resides is preferred.
Professional disciplines may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or (licensed) social worker.
Current BLS certification required.
Three (3) to five (5) years of inpatient experience, preferably in an acute, IRF, or LTACH setting.
