Case Manager at Foothill - Full Time
About The Position
Wasatch Behavioral Health is seeking a Full-Time Case Manager to work at their Foothill Residential Center in Spanish Fork, Utah. The Case Manager will work directly with clients and their families in a supportive, hands-on environment. Requirements
- Bachelor's degree with Social Service Worker (SSW) license or Substance Use Disorder Counselor (SUDC/LASUDC)
- OR Bachelor's degree in any field with willingness to pursue SSW or SUDC/LASUDC licensure
- Applicants under the "working toward SUDC or SSW" pathway must obtain their SUDC or SSW license within 24 months of hire
- Valid driver's license
- Basic understanding of Social Work principles
- Familiarity with social welfare policy and human development
- Ability to problem-solve and maintain a client-centered approach
- Successful completion of a drug screen, motor vehicle record review and criminal background check
- Complete and receive a Criminal Background Clearance (BCI) from Utah State Office of Licensing after hire
- Provide an official copy of licensure or certification upon receipt of a conditional offer of employment

Responsibilities
- Complete client assessments to identify needs and strengths
- Develop individualized care plans and update them as clients progress
- Coordinate services, resources, and community support
- Collaborate with clinicians, treatment teams, and other professionals
- Monitor client progress and maintain accurate documentation
- Help client's problem-solve barriers and build essential life skills
- Communicate effectively with clients, families, and team members
- Approach every interaction with empathy, professionalism, and a client-centered mindset
