About The Position

The Doe Fund is seeking a Case Manager for its newest permanent supportive housing program for homeless individuals, including veterans, living with severe mental illness and/or a history of substance use. This 90-unit residence aims to provide long-term residential care and 24-hour support services in a community environment that promotes resident independence while ensuring their safety and well-being. The program will house 54 individuals referred by the NYC Human Resources Administration (HRA) and 36 units for affordable low-income households. The Case Manager will be responsible for providing supportive case management services and counseling to HRA-referred residents, requiring strong clinical skills and experience with individuals diagnosed with co-occurring disorders and/or trauma.

Requirements

  • Bachelor's degree in Social Work or a relevant field
  • At least 3 years of experience working with homeless individuals diagnosed with serious mental illness and/or substance abuse disorder and/or in trauma settings
  • Expertise in Evidence-Based Practice, including Critical Time Intervention, Motivational Interviewing, and Harm Reduction techniques
  • Excellent clinical and case management skills, including knowledge of benefits in the NYC Metro area
  • Sensitivity to community issues and needs
  • Outstanding interpersonal and communication skills, both oral and written
  • Strong ethical standards, confidentiality, and personal integrity
  • Computer proficiency (i.e., email, Microsoft Word, Excel, and PowerPoint)

Responsibilities

  • Maintain a caseload of individuals diagnosed with serious mental illness and/or substance use disorder and provide supportive case management services to them
  • Develop and facilitate therapeutic groups for residents
  • Maintain resident records in compliance with agency and funding standards
  • Conduct and review assessments
  • Develop and implement individual Service Plans and assist residents with achieving their goals geared toward self-sufficiency
  • Monitor and ensure residents are linked to appropriate treatment and services
  • Provide counseling and crisis intervention as necessary
  • Assist residents in securing and maintaining benefits
  • Maintain a liaison with community services, including hospitals, substance abuse treatment programs, HRA, and mental health programs
  • Monitor and assist residents with rent payments and budgeting
  • Conduct regular unit inspections
  • Educate residents about approaches to mental health and substance use
  • Compile resources on mental health information and treatment strategies
  • Attend ongoing training to enhance the services provided to the residents
  • Assist with the completion of reports as needed
  • Other duties as assigned
