About The Position

We are seeking a full time Case Manager to join our Dual Diagnosis Treatment Team in our Collegeville, PA location. The Case Manager will be responsible for: Collaborating with other members of the team on the implementation of a recovery-oriented person-centered treatment and support plan. Performing initial intakes, assessments, and timeline summaries. Providing intensive therapeutic services in all types of settings, including but not limited to community living arrangements, schools, community settings, and natural support residences. Providing individual and group treatment and support according to each individual’s/family’s needs. Supporting data collection, monitoring interventions, and providing feedback to the treatment team regarding client progress.

Requirements

Bachelor’s degree in Social Work, Psychology, or another human service-related field required.
Valid PA driver’s license with a clean driving record required.
Candidate required to complete clearances and background checks, at Merakey’s expense.

Nice To Haves

Experience in the behavioral health field working with individuals with mental health/developmental disabilities preferred.
