About The Position

Under the direction of the Program Manager/Senior Director of Support Services, the Case Manager, who is a major member of the team provides care coordination, health education, and health promotion services to address barriers to care and provides supportive services to help the patient self-manage his/her illness and ensure that patients have access to and obtain needed medical, psychiatric, and social services. This work is carried out in support of the mission and goals of Ryan Health.

Requirements

  • BSW or BA in Health or Human Services, Health Education, Mental Health, Counseling, or related field.
  • Experience working with people in a health care or social service setting
  • Computer skills.
  • Effective communication and writing skills.

Nice To Haves

  • BSW, MSW or Master’s Degree in a related field.
  • Bilingual (English & Spanish).
  • Demonstrates strong ability to develop relationships with patients with diverse backgrounds (e.g. socio-economic, ethnic, racial, sexual orientation and preference).
  • Demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served (if applicable).
  • Ability to take responsibility for their own learning.

  • Provides coverage for other case managers under the direction of the Supervisor of Case Management.

Responsibilities

  • Ensure coordination and follow-up of medical, psychiatric and social services.
  • Provide reminders for appointments and follow-up contacts for missed appointments
  • Provide home, hospital, or community site visits and escort activities, particularly to hard-to-reach patients, or as indicated in the service plans
  • Provide crisis intervention, assist in developing safety plans and referrals for emergency services when necessary
  • Educate patients regarding early medical intervention, preventive health care, options to reduce/modify stressors, and adherence to treatment regimen and appointment schedules.
  • Provide prevention counseling and education services as needed, for HIV infected, HIV affected, HIV unknown, and HIV-negative patients, and /or significant others.
  • Document all patient encounters and services in a timely manner consistent with grant requirements and agency standards, including the use of the electronic medical record
  • Attend required meetings, supervision regularly, case conferences, care planning, in-service seminars, and professional conferences as assigned.
  • Immediately report any critical/unusual/ potentially dangerous problems or occurrences to the Program Manager/Senior Director of Support Services.
  • Provide educational activities to promote the services of the program, provide educational presentations to staff, and participate in educational activities to outreach in the community
  • Practice accepted infection control measures in accordance with OSHA Bloodborne Pathogens Standards, and the Center’s policies and procedures
  • Perform other duties as assigned.
