Position Summary The Case Manager provides direct, trauma‑informed case management services to individuals experiencing homelessness and to patients enrolled in Recuperative Care (Medical Respite). This role serves as a critical bridge between acute medical care, housing stability, and community‑based support services, guiding clients through complex systems with compassion, structure, and advocacy. The Case Manager conducts assessments, develops individualized service plans, coordinates care across multiple systems, and ensures clients receive the medical, behavioral health, and social supports needed to stabilize and transition successfully.
