About The Position

Serve as part of an interdisciplinary team to deliver case management services using a strengths-based, client-centered approach to adults experiencing homelessness with the goal of securing permanent housing.

Requirements

  • Associate degree in social service required, bachelor’s degree Preferred
  • Excellent verbal and written communication skills
  • Strong organizational skills
  • Ability to negotiate numerous priorities, resolve conflicts, and manage a high degree of communication with multiple parties.
  • Excellent judgement
  • Computer Skills: Proficient in Microsoft Office
  • Ability to learn data base systems, such as CARES

Nice To Haves

  • Experience working within the unsheltered population preferred
  • Experience working with individuals diagnosed with severe and persistent mental illness, substance use and/co-occurring disorders preferred
  • Prior case management in stabilization beds, safe haven, outreach, or supportive housing experience preferred
  • Bilingual (English and Spanish) preferred

Responsibilities

  • Provide case management services to a caseload of approximately 18 clients.
  • Perform intake, assessment, service planning, advocacy, referral and escort services
  • Provide case management; identify short and long-term client-centered goals and help clients achieve them, including linkages to community programs such as medical, psychiatric and substance treatment services.
  • Maintain case records with documentation of services provided and outcomes, according to specific program procedures.
  • Responsible for case notes and service plans using an electronic database
  • Assist clients in applying for, receiving and maintaining entitlements
  • Accompany clients on visits to doctors, entitlement offices or other community services when possible.
  • Serve as liaison between other service providers and community resources
  • Facilitate groups, collaborate with harm reduction specialists and housing specialists
  • Utilize evidence-based practice models including person centered planning, motivational interviewing, trauma informed care, culturally sensitive, and strengths-based approach.
  • Monitor compliance with goals through regular weekly contacts.
  • Perform culturally sensitive intake, assessment, service planning, and advocacy.
  • Work closely with RA staff to enforce rules and regulations for residents.
  • Attend staff meetings, case conferences, training programs and supervision meetings; participate in program evaluation and improvement.
  • Other duties as required.
