The Case Manager works out of the Melrose Housing Program, a 24-hour, 60-bed program serving transitional age youth (TAY) experiencing housing instability. The Case Manager is responsible for supporting youth housing stability and wellness. This includes processing referrals, managing the waitlist, coordinating move-ins and exits, completing intake and documentation, providing intensive case management, creating service plans, connecting youth to resources, assisting with budgeting, benefits, and employment, and ensuring health and safety through crisis intervention and risk assessment.
