About The Position

Assume necessary duties of Hudson House (HH) case manager, work as an integrated team member in all functions of the Hudson House Psychosocial Clubhouse program. Perform duties while actively promoting an environment consistent with MHA’s Cultural Equity Policy and WELCOME Orange.

Requirements

  • Bachelor’s Degree in related field
  • Associate degree in related field with 2 years of relevant experience working with individuals with Serious Mental Illness and/or substance use disorder
  • High School or GED/HSE diploma with 3 years experience in Human Services.
  • Ability to work independently.
  • Effective written and oral communication skills.
  • Must be organized, self-motivated and have the ability to coordinate multiple tasks simultaneously.
  • Ability to exercise sound judgment under crisis situations and to abide by regulations regarding confidentiality.
  • Valid Driver’s License and ability to drive with current driving record acceptable for agency insurance coverage.
  • Must be clear and maintain an acceptable record under the Office of Mental Health (OMH) mandated criminal background check process.
  • Must be clear and maintain an acceptable record under the Medicaid Fraud & Abuse clearance process.

Responsibilities

  • Provide social rehabilitative services to a caseload of 20-25 adult program participants with a mental health diagnosis and/or substance use disorder.
  • Services include the development of person-centered service plans, teaching social skills in group and individual settings, and supporting overall social development, community integration, and inclusion while at program.
  • Serve as an advocate for program participants’ rights.
  • Assist the Hudson House team with crisis intervention and response as needed.
  • Display active, meaningful, and appropriate interactions with members at all times.
  • Facilitate daily scheduled rehabilitative groups and activities both onsite and in the community.
  • Assist in the kitchen with meal preparation, following food safety policies, to ensure that members receive breakfast, lunch, and afternoon snack.
  • Maintain all Hudson House forms such as: Transportation logs, sign in / Sign out sheets for meals, Attendance Records.
  • Actively promote and facilitate members’ wellness and recovery.
  • Complete all required program paperwork in an accurate, thorough, and timely manner.
  • Transport members using an agency vehicle to and from program, and in the community to and from travel activities.
  • Provide data entry into Foothold Technology Electronic Health Records System.
  • Support the overall operations of the building including light snow removal from agency vehicles and main walkways.
