About The Position

Requirements

  • A professional with a strong understanding of case management activities, including linkage, referral, advocacy, and crisis intervention.
  • Someone with excellent organizational and time-management skills who can effectively plan and prioritize a flexible schedule.
  • An effective communicator with a proven ability to build positive working relationships with clients, families, and community partners.
  • A resourceful problem-solver who can identify and connect clients with the services they need.
  • Bachelor’s Degree in Human Services field of study (required)
  • Successful completion of HCBH pre-employment screening and background check.
  • Has the ability to communicate effectively orally and in writing.
  • Has a valid driver’s license and eligible for HCBH insurance coverage(s).
  • Proficient in Microsoft Office: Word, PowerPoint, Excel, Outlook.
  • Ability to conceptualize, develop and implement programs to their completion in a fast-paced environment.

Nice To Haves

  • 2 years experience in Medical and/or Behavioral Health (preferred)
  • High degree of organizational skills and attention to detail.

Responsibilities

  • Provide client-centered support: Help individuals navigate complex service systems and connect them with community resources that meet their specific needs.
  • Manage and coordinate care: Develop and monitor service plans, ensuring clients receive the right support and make progress toward their goals.
  • Build and maintain a resource network: Continuously research and stay up-to-date on available resources in the community to best serve clients, their families, and our partners.
  • Document and monitor progress: Maintain accurate and timely documentation of client progress in our Electronic Health Record (EHR) system.
  • Collaborate with the team: Work closely with the Clinical Director, therapists, and other staff to ensure seamless service delivery and address any client needs or conflicts.
