About The Position

The Case Manager works at HomeFirst’s Emergency Interim Housing (EIH) Cherry program in San Jose, supporting unhoused single adults as they transition from emergency interim housing toward stable permanent housing. The program provides a safety-focused, client-centered continuum of services, including emergency interim housing, a transition period, and bridge housing. The site offers private sleeping units with restrooms and showers, shared community facilities, supportive services, and on-site security, and operates 24 hours a day, seven days a week. The Case Manager documents services and assessments in HMIS, monitors participant needs and program compliance, and helps participants transition to the next appropriate level of care.
