About The Position

Valley Woods is a place where consistent support can turn everyday progress into greater independence. As a Case Manager, you’ll work alongside clients to build practical skills, navigate challenges, and move toward goals that matter to them, providing the encouragement and resources that help create lasting stability. Case Managers provide psychosocial rehabilitative services as indicated by the client’s care plan to support clients in meeting their individualized goals. Case Managers are responsible for utilizing evidence-based practices, meeting fiscal responsibilities, and ensuring that regulatory compliance and organizational standards are met.

Requirements

High School diploma or equivalent
None (experience)
Case Manager certification (post hire as needed - required within 120 days of start date)
CPR certification (Post-hire)
Valley de-escalation certification (Post-hire)
Minimum age of 21 and a current driver’s license (for driving positions)

Responsibilities

Provides psychosocial rehabilitative services as described by the Medicaid manual
Supports clients in following individualized care plans and identifying case management goals
Completes documentation on services provided within Valley’s standards
Meets productivity expectations as assigned by supervisor
Distributes medications as necessary
Transports clients to appointments as necessary
