About The Position

The Community Service Board of Middle Georgia's Adult Outpatient program is seeking a qualified individual to serve as our next Case Manager. ADSS Case Management consists of providing environmental support and care coordination considered essential to assist a person with improving his/her functioning, gaining access to necessary services, and creating an environment that promotes recovery as identified in his/her Individual Recovery Plan (IRP). The focus of the interventions includes assisting the individual with: 1) developing natural supports to promote community integration; 2) identifying service needs; 3) referring and linking to services and resources identified through the service planning process; 4) coordinating services identified on the IRP to maximize service integration and minimize service gaps; and 5) ensuring continued adequacy of the IRP to meet his/her ongoing and changing needs.

Requirements

  • High School Diploma/GED Equivalent
  • Valid Georgia Driver’s License
  • Effective verbal and written communication skills
  • Strong interpersonal skills and the ability to work effectively with diverse communities
  • Ability to work independently and in collaboration with others
  • Experience with Microsoft 365 Office Products
  • Ability to organize, prioritize and meet deadlines accordingly

Nice To Haves

  • Bachelor’s degree in a related field (psychology, social work, counseling, etc.)
  • Previous experience in case management or behavioral health

Responsibilities

  • Monitor and document consumer progress, ensuring treatment plans and interventions are followed and progress notes are completed within agency timelines
  • Coordinate care activities and assist individuals as they move between services and support
  • Share necessary care information with providers, physicians, residential programs, and other identified support
  • Act as a liaison to connect consumers with internal resources and outside community supports, encouraging independent use of resources when possible
  • Collaborate with consumers, families, providers, and community partners to ensure continuity and quality of care
  • Maintain compliance with agency, state, and federal documentation standards
  • Participate in required training, monthly staff meetings, and agency/court meetings as requested
  • Demonstrate teamwork, professionalism, and organizational commitment by working cooperatively with colleagues, showing persistence in problem-solving, and adapting to new processes
  • All other duties as assigned.
