About The Position

AtlantiCare Rehabilitation Hospital, in partnership with Select Medical, is opening a new 60-bed medical rehabilitation hospital in Galloway, New Jersey, in August 2026. This hospital will serve patients in southeastern New Jersey and will be led by a physiatrist. The team will include rehabilitation nurses, physical, occupational, and speech-language therapists, dietitians, case managers, and other clinical and support staff. The hospital is dedicated to providing exceptional and compassionate care for individuals with a range of challenges including brain injuries, spinal cord injuries, neurological disorders, orthopedic issues, amputation, and multiple traumas. Requirements

  • Current Licensure per state guidelines in clinical or related discipline OR a Bachelor's or Master's in a health or human services discipline.
  • Previous experience in Case Management and Discharge Planning is required.

Nice To Haves

  • CCM Certification Preferred.

Responsibilities

  • Coordination of health care decisions using a systematic approach to assure treatment plans that improve quality and outcomes.
  • Coordination of care across the continuum.
  • Promotion of cost-effective care within the allotted time frame.
  • Assuring payments of hospital-based services meeting patient-related utilization management criteria.
  • Implementation of safe and appropriate discharge plans.
  • Assessment of the psychosocial needs of the patient and provision of intervention as part of the discharge planning process.
