About The Position

Provides assessment, service coordination, and counseling to geriatric clients eligible to receive multiple services in the community. This role involves studying and assessing existing resources, providing direct case management services, preparing reports, and maintaining client files. The Case Manager will also collaborate with external agencies and internal supervisors to ensure client needs are met. Continuous professional development through training and supervision is expected, along with adherence to organizational policies and mission.

Requirements

  • Bilingual Korean/English speaking ability.
  • Bachelor's Degree required.
  • One year related experience and/or training.
  • Working knowledge of Microsoft Office Suite and other technology.
  • Excellent communication and listening skills.
  • Excellent customer service skills with a focus on treating clients with respect and dignity.
  • Able to multi-task and work independently with great attention to detail.

Responsibilities

  • Study and assess the strengths and weaknesses of existing resources.
  • Provide case management services to clients, including intake, emergency back-up, ongoing advocacy, in-home assessments, financial management, crisis intervention, and information and referral.
  • Prepare reports and disseminate information.
  • Provide statistical reports and documentation as required.
  • Maintain proper client files with appropriate documentation and information.
  • Maintain contact with representatives of other organizations to exchange and update information on resources and services available.
  • Form and maintain formal relationships with agencies to secure services and benefits for clients.
  • Coordinate client care with field supervisors and nurses as needed.
  • Participate in in-service training, conferences, seminars, and use journals to update skills, as well as receive consultation and supervision.
  • Attend departmental and organizational meetings and regular supervisory sessions.
  • Keep supervisor informed of activities, needs, and problems related to client services.
  • Perform other duties as needed or requested.
  • Adhere to the organization’s standards by completing annual mandatory trainings.
  • Supports the organization’s mission by striving for excellence and maintaining positive interpersonal relationships with co-workers.
  • Adheres to the organization’s policy in regards to absenteeism and appearance.
