About The Position

Volunteers of America Upstate New York's continuum of residential programs offer homeless & low-income individuals and families safe housing and support services. We provide long-term solutions to the problems of homelessness, addiction, incarceration, and poverty by helping the people we serve overcome obstacles and develop healthier, more self-sufficient lives. The case manager works with residents of our emergency family shelter and is responsible for assisting residents to develop and achieve their individual goals and ensure a successful transition from homelessness to permanent housing. All interactions are conducted with in accordance with Agency guidelines and best practices in the social work field.

Requirements

  • Bachelors Degree/ in Human Services or Social Work or equivalent combination of education and experience
  • Minimum two (2) years experience in human services, preferably with families in crisis
  • Clean NYS drivers license.
  • Availability to work a minimum of 2 evenings per week and 1-2 weekends per month; flexible hours as required to meet resident needs.
  • COVID 19 Vaccination
  • Knowledge of DHS and other local resources in the community particularly housing resources.
  • Interacts in a polite, courteous, professional manner.
  • Computer literate.

Responsibilities

  • Provide emergency housing and services in a safe, hospitable environment (24 hour staffing), emphasizing harm-reduction procedures
  • Completes a computerized intake, assessment and develops an individualized service plan for each client/family unit within 48 hours of arrival at the shelter.
  • Establishes and maintains good working relationships with related agencies to ensure VOAWNY maintains a comprehensive referral network for clients.
  • Assist families/clients in obtaining identification and other eligibility requirements necessary to ensure assistance from the Department of Human Services or other sources.
  • Uses evidence-based programming to help families address problems and become stabilized; Provide crisis intervention (on-going as needed)
  • Creates transition plans for families to locate and secure affordable permanent housing.
  • Provides life skills training and helps the heads-of-household to conduct job searches as appropriate.
  • Transport clients to important appointments when necessary.
  • Establishes and maintains accurate client records for in compliance with funder requirements, HMIS and VOAWNY guidelines.
  • Participate in a minimum of one weekly case conference.
  • Meets with residents at least weekly to assist them to achieve their goals.
  • Conduct weekly 30 minute community meetings (all residents in shelter)
  • Coordinates weekly activities for families, including parenting skills groups, and offer family reunification services as needed.
  • Provides referrals as appropriate for treatment, medical care, mental health services, etc.
  • Perform other duties as assigned.
