JOB SCOPE: As a member of the CCBHC team and under the supervision of the Recovery & Support Supervisor, the Case Manager is responsible for addressing all member needs, including developing/updating care plans. The Case Manager must be skilled in engagement, advocacy, crisis intervention and stabilization, and developing community linkages. The Case Manager is required to provide services both in the office and in the community, to ensure that a consumer’s needs are met. The Case Manager will work with community agencies/health care providers and others on the behalf of individuals to ensure access to services needed to improve their ability to function in the community and increase wellness self-management. CCBHC Case Manager is responsible for participating in mobile crisis team, with rotating on-call coverage to respond to community-based crises outside of clinic operating hours.
