About The Position

The Steven A. Cohen Military Family Clinic at Alaska Behavioral Health provides confidential, high-quality behavioral healthcare services and local resource connection to post-9/11 veterans, including National Guard and Reserves, their families, active-duty service members, and the families of active-duty service members. This includes spouses or partners, children, parents, siblings, caregivers, and others. The Case Manager will work as part of an interdisciplinary team to support coordinated, client-centered care. This position helps clients identify needs, reduce barriers to care, and connect with community, military-connected, VA, nonprofit, and social service resources throughout the state of Alaska. The Case Manager will work closely with clinicians, group therapy providers, the Fairbanks Case Manager, and Lead Case Manager to support timely and culturally responsive care.

Requirements

  • Minimum of a bachelor’s degree in social work, sociology, psychology, or related field is required.
  • Experience in case management, care coordination, social services, behavioral health, or related work.
  • Strong communication, organization, documentation, and follow-through skills.
  • Comfortable working collaboratively with clinicians, case management staff, and clinic leadership.
  • Experience working with military populations and/or veterans, or a strong interest in serving this community.
  • Working knowledge of Outlook, Word, Excel, and other Microsoft applications.
  • Committed to trauma-informed, culturally responsive, and strengths-based care.

Nice To Haves

  • Master’s degree is preferred.
  • Background with Electronic Health Records (EHRs) a plus.

Responsibilities

  • Provide case management support to clients and families enrolled in services at the clinic.
  • Review client resource needs and assist with resource recommendations, warm hand-offs, and community connections.
  • Respond to case management requests from clinic staff on behalf of clients.
  • Collaborate with case management team to support timely follow-up and coordination of case management needs.
  • Maintain accurate documentation and administrative tracking of client care.
  • Support clinic groups through recruitment, advertising, and occasional psychoeducation group facilitation.
  • Participate in monthly staff meetings, case management meetings, and required training.
  • Provide limited community outreach as needed.
