About The Position

Under the supervision of the Director of Wallis House, the Wallis House Case Manager provides on-site case management services and support for residents. The Case Manager coordinates care with intra-agency staff, PATH, LAHSA, referring agencies and the community at large. In addition, they serve a caseload of residents with children. The Case Manager is an energetic self-starter who is able to function comfortably in a team environment and relates well to co-workers, volunteers, and external representatives. This position requires flexibility and the ability to manage time and multiple tasks. The individual in this position must be able to assume a wide range of responsibilities, provide timely responses to requests and deadlines and works well under pressure.

  • High School Diploma or equivalent
  • Experience working with individuals experiencing homelessness
  • Valid CA driver’s license
  • Department of Justice/Fingerprint clearance
  • Satisfactory completion of health screening
  • Tuberculosis clearance
  • Proof of legal eligibility to work in the United States

Responsibilities

  • Provide targeted case management as assigned for resident adult and children who reside at the Wallis House.
  • Conducts enrollments/exits In HMIS and EXYM. Creates Individualized Service Plan (ISP) that addresses housing goals, maintaining/increasing income and other personal goals identified by the client including medical, mental health, substance use, financial resources, vocational, and social support needs.
  • Provides referrals, linkages, information, and support to resources that help clients to achieve their ISP goals (this includes collecting documentation to secure housing).
  • Completes program specific assessments (Annual Assessment/ Status Assessment)
  • Evaluates strengths and challenges to addressing short term and long-term goals, conducts a 90-day review with the client after initial intake
  • Case Manager is a family’s Point of Contact for any additional referring agencies and creates relationships with other social workers to ensure the success of the family.
  • Case Managers are connected to the Family’s Housing Navigator and must be in constant communication with Housing Navigator to ensure the progress of the Family’s Service Plan. Case Manager are also responsible for referring cases to Housing Navigation program once a family is Housing ready.
  • Assist with conflict resolution in coordination with Resident Case Navigator always aiming to maintain a fair, positive, and supportive culture.
  • Facilitate enrollments for new residents, including collecting data, inspecting resident belongings, and orienting residents to house expectations and guidelines.
  • Complete and submit all documentation to Exym, HMIS and LAHSA including but not limited to progress notes, LAHSA required forms, outcome measures and goals in a timely manner.
  • Works to promote and support justice, equity, diversity, and inclusion at all levels in the organization.
  • Works to promote a trauma-informed work environment
  • Meet weekly with supervisor and consult as necessary when regarding case assistance. Case Manager’s are mandated reporters of Child Abuse/neglect.
  • Works collaboratively and cooperatively within a multidisciplinary team
  • In collaboration with team members, create and maintain Crisis Safety Plan for families
  • Maintain effective working relationships, and coordinate care, with intra-agency staff, County workers, school personnel, referring agencies, and the community at large
  • Sensitive and responsive to the cultural differences among the agency’s client population
  • Participate in all agency required meetings and trainings
  • Adheres to all applicable Federal and State regulations for confidentiality, HIPAA, mandated reporting of suspected child or elder abuse
  • Works to promote and support justice, equity, diversity, and inclusion at all levels in the organization.
  • Works to promote a trauma-informed work environment.
  • Actively participate in a quality improvement project
