About The Position

The WorkPlace is an organization dedicated to strengthening the workforce through innovation and action. The Platform to Employment (P2E) Youth Program is seeking a Case Manager to provide trauma-informed, individualized support to disconnected and at-risk youth aged 18-26 who are not currently in school or employed. This role involves helping participants overcome personal and systemic barriers through case management, job readiness training, educational guidance, and wraparound services. The position requires regular travel to various locations to engage with participants and community partners, aiming to guide youth toward long-term success in employment, education, or vocational training.

Requirements

  • Excellent intrapersonal skills with the ability to build rapport, motivate, and maintain positive relationships with youth and colleagues.
  • Highly organized with the ability to manage multiple cases, meet deadlines, and adapt to changing priorities.
  • Proficiency in Microsoft Excel for tracking participant progress, data entry, and reporting.
  • Comfortable working independently and in a team across multiple sites.
  • Commitment to assisting at-risk and disconnected youth in overcoming barriers to achievement and success in their personal and work lives.
  • Excellent verbal, presentation, and written communication skills.
  • Demonstrates strong cultural competence and the ability to work effectively with individuals from diverse backgrounds, fostering an inclusive and respectful workplace.
  • Bachelor's degree or equivalent from a four-year college in Human Services, Education, or Business Management, and/or three years of related experience and/or training; or equivalent combination of education and experience.
  • The ability to understand more advanced computer skills and be proficient in the advanced knowledge of software such as Microsoft Word, Excel, PowerPoint, and Access as well as data management systems.

Nice To Haves

  • The ability to read, speak and/or interpret Spanish is desired.
  • Preference given for training in Professional Accredited Resume Writer (PARW).

Responsibilities

  • Prepare and maintain records and case files, including documentation such as clients' personal and eligibility information, services provided, narratives of client contacts, and relevant correspondence.
  • Provide one-on-one case management and mentorship to disconnected and at-risk youth aged 18-26.
  • Support youth in addressing barriers such as housing instability, mental health needs, justice involvement, and family challenges.
  • Develop a mutually agreed upon individualized Employment Plan & Service Plans to identify the participants' short-term and long-term objectives, update plan as needed and monitor their progress.
  • Identify and initiate appropriate referrals that will strengthen the participants' potential for job placement and /or post-secondary school acceptance.
  • Provide information or refer individuals to public or private agencies or community services for assistance.
  • Maintain close contact with clients during job training and placements to resolve problems and evaluate placement adequacy.
  • Monitor and record clients' progress to ensure that goals and objectives are met.
  • Develop and maintain a database of participant activities with appropriate documents and required paperwork, i.e., forms, case notes, work experience, and volunteer opportunities.
  • Submit reports and review reports or problems with superiors.
  • Accumulate program data for regular internal and external reporting, and program evaluation, as needed.
  • Represent the workplace on committees and tasks forces while connecting/referring and coordinating the proper outreach and case management services needed for participants.
  • Ensure a strengths-based, culturally competent, and youth-focused approach to all services.
  • Other duties may be assigned.
