Under the direct supervision of a Social Worker (MSW), the job of a Case Manager is to regularly meet with youth participants, helping them create individual goals and tracking their accomplishments, with the end goal of preventing youth involvement with violence. The Case Manager will also engage family members and caretakers when appropriate. The Case Manager will conduct outreach to the community and internally; build rapport with families and targeted youth; provide case management services in accordance with the program requirements and administration's directions & guidance; establish and maintain records; compile and distribute materials and reports; respond to inquiries from a variety of internal and external sources; and do other related work as needed.
