About The Position

Under the direct supervision of a Social Worker (MSW), the job of a Case Manager is to regularly meet with youth participants, helping them create individual goals and tracking their accomplishments, with the end goal of preventing youth involvement with violence. The Case Manager will also engage family members and caretakers when appropriate. The Case Manager will conduct outreach to the community and internally; build rapport with families and targeted youth; provide case management services in accordance with the program requirements and administration's directions & guidance; establish and maintain records; compile and distribute materials and reports; respond to inquiries from a variety of internal and external sources; and do other related work as needed.

Requirements

  • Some college in social work, psychology, sociology, human services or related field.
  • Bachelor’s degree in social work, psychology, sociology, human services, or related field.
  • Two years of increasingly responsible experience working with youth populations, particularly in areas with high rates of violence.
  • Knowledge of working with youth in a trauma-informed environment.
  • Knowledge of assessing youth wellness, needs, and abilities and using their knowledge to develop plans.
  • Knowledge of listening to youth/clients’ concerns and providing mentorship.
  • Knowledge of recording youth/clients’ progress, including charting referrals, home visits, and other notable interactions.
  • Knowledge of organization and coordination of specialized and responsible clerical functions.
  • Knowledge of modern office methods and equipment, including electronic record management and filing systems, computer terminal operational processes, and telephone techniques.
  • Knowledge of correspondence and report writing.
  • Knowledge of English usage, grammar, spelling, and punctuation.
  • Knowledge of basic methods and techniques of organization and planning, following a schedule, creating a case plan for each youth, and ensuring requirements for each youth are followed.
  • Ability to work effectively with youth, showing compassion and ability to relate to youth/clients with a wide range of needs.
  • Ability to motivate youth to follow established plans.
  • Ability to think critically and problem-solve.
  • Ability to flex and change plans if not getting desired results.
  • Ability to manage multiple cases at once.
  • Ability to learn, interpret, and apply legal mandates, policies, rules and regulations, and operational procedures.
  • Ability to compose correspondence independently.
  • Ability to establish and maintain comprehensive and accurate files and records.
  • Ability to prepare concise and detailed reports and documentation as needed.
  • Ability to drive agency vehicles to transport youth and families to community/club events, scheduled activities, appointments, etc.
  • Ability to understand and carry out oral and written directions.
  • Ability to apply and explain policies, procedures, rules and regulations.
  • Ability to operate a computer terminal to enter data, maintain records, and generate reports.
  • Ability to plan and organize work and meet schedules and deadlines.
  • Ability to establish and maintain cooperative working relationships.

Nice To Haves

  • Enrolled in an accredited master’s degree program in social work preferred.
  • Bilingual in English and Spanish preferred.

Responsibilities

  • Attends site and in-service meetings to convey and gather information required to perform functions.
  • Assists with participant outreach by preparing and distributing outreach material, making presentations, phone calls, and scheduling meetings.
  • Maintains various manual and electronic documents/reports, files, and records to provide up-to-date information and historical reference per established administrative guidelines and legal requirements. This may include participant information.
  • Assists with assessing participants’ needs, situations, personal strengths, and support networks to work with youth and determine appropriate goals.
  • Coordinates with appropriate organizations and services including, but not limited to, social services, physical and mental health services, housing support, employment education, and substance use disorder treatment.
  • Monitors and tracks milestones such as employment acquisition and educational progress.
  • Responds to inquiries from various internal and external parties (e.g., schools, partners, government agencies, the general public, students, etc.) to provide information, facilitate communication among parties and provide direction.
  • Meets with youth and families in compliance with program requirements, researches and refers participants to community resources.
  • Tracks youth participation in positive activities and provides support to engage youth in these activities.
  • Responsible for all tasks related to holding case management meetings with youth and families.
