About The Position

LifeStream is seeking a Case Manager to join their team. This role involves managing confidential records, coordinating consumer training, ensuring adherence to program guidelines, and maintaining communication with various stakeholders including DDS, parents/guardians, and program managers. The Case Manager will also be responsible for billing, quality assurance checks, and providing direct services when needed. A commitment to community inclusion and flexibility in scheduling are essential.

Requirements

  • Maintenance of an acceptable CORI
  • Valid Driver’s license
  • Negative drug screen
  • Use of personal vehicle for work purposes
  • Initial physical exam with Mantoux testing. Mantoux screening annually thereafter. (ABI, MRC programs only)

  • Maintain flexibility with weekly schedule in order to accommodate program needs.
  • Demonstrated history of quality work performance and teamwork in previous positions
  • Proven Leadership skills
  • 1-3 years experience in the field of DDS.

Responsibilities

  • Review HCP forms, enter information into HCSIS and appointment schedule and forward to DDS and Day program.
  • Maintain confidential records and medical records (including filing of paperwork) of all consumers who participate in the program in accordance with LifeStream policy.
  • Maintain updated Health Care Records for all individuals related to medication changes, medical appointments, diagnosis and medical history.
  • Review monthly data sheets submitted by Personal Advocates for accuracy, content, etc.
  • Completes weekly billing for assigned programs of the Community and Family Supports Division.
  • Maintain updated Fact Sheets and Appointment Schedules for all clients in the program.
  • Ensure consumer training occurs according to established guidelines (Fire Safety, DPPC, HR and Voluntary Checklist) and appropriate documentation is maintained
  • Maintain regular contact with program manager to ensure appropriate implementation and progress on consumer ISP goals and objectives
  • Maintain contact with DDS, and Parents/Guardians as needed.
  • Act as a member of the team offering input into all programmatic services, schedules, systems, etc.
  • Make recommendations regarding program quality, consumer choices and provide general input to program coordinator as needed.
  • Provide direct services/coverage when required and requested by program manager.
  • Makes recommendations to program coordinator relevant to staff training needs.
  • Ensure that staff decisions are based upon consumer choice and empowerment.
  • Demonstrates flexibility in meeting program needs.
  • Conduct Quality Assurance Checks, at least quarterly, on assigned program records.
  • Set up medical and confidential files for new program participants.
  • Update Emergency Fact sheets and clinical database as pertinent information changes.
