About The Position

Volunteers of America Northern California & Northern Nevada (VOA-NCNN) is seeking a full-time, Permanent Supportive Housing (PSH) Case Manager to provide on-site services at Pacific Crest Commons in Truckee, CA. This role supports tenants in 10 PSH units funded through No Place Like Home (NPLH), assisting adults living with serious mental illness who have experienced or are at risk of homelessness. The program operates under Housing First principles, where services are voluntary and success is measured by tenants maintaining their homes. The Case Manager will foster an environment of dignity and respect, ensuring participants are seen as whole individuals. The work schedule is Monday - Friday, 8:30 AM - 5:00 PM.

Requirements

  • A minimum of two (2) years’ social work-related education from an accredited academic institution or (3) years of experience working with the homeless population or alcohol/drug recovery programs is required, or a combination thereof.
  • Demonstrated ability in record keeping.
  • Proficient written and verbal communication skills.
  • Knowledge of problems unique to women, minorities, persons with disabilities, persons with a history of substance use, individuals with low to no income, and the general unhoused population.
  • CPR and First Aid training within ninety (90) days of employment date and recertification as necessary.
  • A valid driver’s license.
  • Reliable transportation.

Responsibilities

  • Develop a case management plan to assist PSH participants in developing greater self-reliance and independence.
  • Link participants to education, employment, budgeting strategies, and money management.
  • Address bio-psycho-social issues that may lead to homelessness, criminal behavior, or non-productive lifestyles.
  • Create and maintain an environment where dignity thrives alongside structure, where participants feel seen as whole people rather than problems to solve.
  • Hold a welcome visit with each new tenant in their first week, then complete a strengths and needs assessment.
  • Build a written, tenant-driven Individualized Service Plan (ISP) within 30 days of move-in, including a Crisis Response Plan, and review it at least every 90 days.
  • Carry out weekly in-unit check-ins to track ISP goals and unit condition.
  • Serve as the central hub of each tenant's care team, coordinating internal and external providers.
  • Use Motivational Interviewing and strength-based, trauma-informed approaches; arrange interpreters as needed.
  • Maintain positive relationships with program participants, landlords, Volunteers of America staff, and all community service agencies.
  • Create and maintain case management files on each participant, including personal information, educational and job skills, short-term and long-term plans to become self-sufficient, and documentation of progress and assistance in removing obstacles to set goals.
  • Provide evaluations of individual clients’ needs to maintain their housing and document all follow-up and placements.
  • Provide crisis management as needed and mediate conflicts with fellow participants, community partners, and neighbors.
  • Complete all discharge paperwork in a timely fashion.
  • Complete budget and housing stabilization plans with participants.
  • Provide resource information and appropriate case planning specific to any participant who has a physical or mental impairment.
  • Participate in Volunteers of America and outside training sessions.
  • Perform other reasonably related duties as assigned by the Program Manager.
  • Conduct monthly house meetings.
  • Arrange or facilitate participant transportation, when needed.
  • Responsible for assistance with client development, tenancy support, and prevention of protection.
  • Liaison between clients and community service agencies to meet client needs.
  • Maintain a list of all available community services and providers.
  • Assure a safe, non-judgmental environment for the clients.
  • Maintain positive, professional relationships with clients and staff.
  • Track and monitor signs of housing instability (late rent, lease violations, neighbor relations, unit condition) and conduct intervention early.
  • Coach tenants on budgeting, lease terms, unit upkeep, daily routines, meal planning, and hygiene, at their own pace.
  • Help with move-in (utilities, household supplies, orientation), lease renewals, and reasonable accommodation requests.
  • Respond within 1-3 business days to any lease violation or eviction notice with a written stabilization plan.
  • Meet with property management at least monthly and mediate landlord–tenant and tenant–tenant conflicts.
  • Maintain files on each participant's short- and long-term plans to maintain and improve self-sufficiency, documenting progress and assistance in removing obstacles to set goals.
  • Provide routine documentation of coordination and follow-up of all areas and collecting financial information.
  • Keep records confidential under HIPAA and 42 CFR Part 2.
  • Maintain participant data in HMIS, Coordinated Entry, and Credible. Document services and outcomes for County reporting.
  • Provide records and services in compliance with agency policies, regulatory, and funding source requirements.
  • Maintain daily, clear, and thorough communication with the Program Manager on all areas of responsibility.
  • Responsible for all areas of daily program maintenance in compliance with company policies.
  • Document and report all client concerns and/or infractions that impact program security.
  • Secure approval for exceptions to standard policy.
  • Notify management of potential workflow problems and resource needs necessary to attain performance standards.
  • Adhere to staffing schedules to provide adequate/safe staffing coverage and to accomplish program objectives.
  • Provide emergency shift coverage as needed.
  • Make warm referrals to NCBH therapy, case management, and psychiatry; offer ongoing, low-pressure invitations to tenants not yet engaged.
  • Support appointments with scheduling, reminders, transportation, and follow-up; join NCBH care coordination meetings.
  • Screen tenants for primary care at move-in and link them to local providers, dental, vision, and specialty care; coordinate CalAIM Enhanced Care Management where eligible.
  • Complete a benefits screening within 30 days of move-in and assist with SSI/SSDI, Medi-Cal, CalFresh, IHSS, Veterans' benefits, and representative payee options.
  • Maintain a current file of Truckee-area resources available year-round.
  • Work to develop and maintain strong ties with NCBH, Nevada County HHSA, and local providers.
