About The Position

Care Lync is looking to hire a full-time Case Manager to provide the activities necessary to meet the needs of the clients assigned to the consolidated care team. Responsibilities

Provide the activities necessary to meet the needs of the clients assigned to the consolidated care team.
Discern the disabilities and accompanied needs of our participants as well as helping them achieve their goals.
Make a plan for each participant and modify it annually or when there is a significant medical or social change.
Communicate with other individuals and entities to ensure seamless coordination between physical, behavioral, and support services.
Develop and maintain professional working relationships with participants and their families.
