About The Position

SSM Health Rehabilitation Hospital, a joint venture between Select Medical and SSM Health, is seeking a Full-Time Case Manager for their Richmond Heights, MO location. This role operates Monday through Friday, with 8-hour days. The hospital specializes in providing exceptional and compassionate care for individuals with a wide range of complex medical conditions, including brain injuries, spinal cord injuries, neurological disorders, orthopedic issues, amputations, and multiple traumas. Requirements

• Current Licensure per state guidelines in clinical or related discipline (RN Preferred) OR a Bachelor's or Master's in a health or human services discipline.

Nice To Haves

• Previous experience in Case Management and Discharge Planning preferred.
• CCM Certification Preferred.
• Acute Rehab experience preferred.
• RN/PT/OT/SLP/RT/SW preferred.

Responsibilities

• Coordination of health care decisions using a systematic approach to assure treatment plans that improve quality and outcomes.
• Coordination of care across the continuum.
• Promotion of cost-effective care within the allotted time frame.
• Assuring payments of hospital-based services meeting patient-related utilization management criteria.
• Implementation of safe and appropriate discharge plans.
• Assessing the psychosocial needs of the patient and providing intervention as part of the discharge planning process.
