About The Position

WHO WE ARE: The Children's Home partners with children, families and communities throughout New York State to inspire hope, develop skills, and cultivate healthy relationships for positive futures. $58,000-62,000 Position Summary: As part of the Critical Time Transition Program (CTTP), the Case Manager provides intensive Critical Time Intervention (CTI) services for youth ages 11–17 with complex mental health needs and their families/caregivers, supporting transitions from higher levels of care to community settings. CTI is a phased, time-limited care management model emphasizing early and assertive engagement, trauma-informed practice, strengths-based approaches, resilience, psychiatric rehabilitation, positive behavioral strategies, and family/caregiver skill-building.
