About The Position

The Case Manager is responsible for coordinating the daily schedules of youth residents, managing their treatment-related activities, and serving as a liaison between the facility and external agencies, including child welfare. This role ensures residents receive consistent, individualized care by managing appointments, coordinating visits, and transporting clients as needed. Hope campus is committed to creating a safe, trauma-informed residential environment where youth are supported through therapeutic relationships, emotional regulation, and the use of most appropriate interventions for each young person.

Requirements

  • A bachelor’s degree in social work, counseling or related field
  • A minimum of two (2) years' experience in counseling, childcare, social work, and/or in providing therapeutic support services to children and families
  • An equivalent combination of education, training, and experience

Responsibilities

  • Develop, update, and manage individualized daily and weekly schedules for residents.
  • Serve as a primary liaison with child welfare agencies, schools, and other external stakeholders.
  • Coordinate and schedule residents’ appointments, including medical, therapeutic, legal, and educational engagements.
  • Facilitate and supervise family or guardian visitations as per treatment plans and legal requirements.
  • Transport clients to appointments or activities using facility-approved vehicles while ensuring safety protocols are followed.
  • Document all client interactions, appointments, and visitations in compliance with organizational policies and licensing requirements.
  • Provide updates and reports to the treatment team, child welfare agencies, and other stakeholders as necessary.
  • Participate in regular case reviews to ensure care plans are current and effective.
  • Assist with crisis management, including de-escalation of residents if necessary.
  • Responsible for creating and facilitating youth treatment plans, discharge plans, and discharge summaries.
  • Responsible for Epic billing for Therapeutic Behavioral Service notes.
  • May be tasked with leading life-skills/TBS groups.
  • Represent the facility in community outreach, networking, and partnership-building activities.
  • Participate in special projects or initiatives as assigned by senior leadership.
  • Act as a liaison between staff, residents, families, and other stakeholders.
  • Other duties as assigned
