About The Position
Under direct supervision, serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community. Shifts Available: Full-Time | DAYS | Shifts: 8:30 AM -5:00 PM | Days: Monday - Friday. Visions is a Full Service Partnership Intensive Case Management (FSP-ICM) program serving up to 191 adults with serious mental illness who require a higher level of community-based behavioral health support to achieve stability, recovery, and independence. The program utilizes a multidisciplinary team approach and operates under a "whatever it takes" philosophy, providing individualized, recovery-oriented services tailored to each participant's unique needs and goals.
Requirements
- High School Graduation or G.E.D. equivalent and three (3) years of direct services in mental health or community services; OR Associate’s degree and two (2) years of experience; OR Bachelor’s degree in Social Services
- At least 1 year of direct service experience providing assistance as a Vocational Specialist
- Valid and current driver’s license
- Personal vehicle insurance with your name listed as a driver
- Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
- Willingness to use the company vehicle to drive clients to appointments and groups, etc.
Responsibilities
- Serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community.
- Provide assistance as a Vocational Specialist.
- Drive clients to appointments and groups using a personal or company vehicle.
