About The Position

Person in this position will function as an interdisciplinary team member, assisting other profession/non-professional personnel in a rehabilitation treatment program. Peer Specialist performs a variety of supportive tasks that include assisting Veterans in articulating their goals for recovery, learning, and practicing new skills, helping them monitor their progress, assisting them in their treatment, modeling effective coping techniques and self-help strategies based on the peer specialist’s own recovery experience, and supporting them in advocating for themselves to obtain effective services. Essential Job Functions include those listed below.

Requirements

  • Ability to operate a computer and complete documents in Microsoft Word program formats or the ability to learn.

  • Must be culturally/linguistically sensitive to populations served.
  • Must be able to successfully obtain and maintain a criminal background clearance
  • Valid driver’s license and ability to obtain and maintain WestCare’s vehicle insurance is required
  • CPR Certification, First Aid Certification, and an annual Tuberculosis test.
  • Experience working with individuals suffering from mental health and substance abuse disorders is required.
  • High School Diploma or equivalent is required.

Nice To Haves

  • Military/Veteran is preferred.
  • Two (2) years of experience in counseling, therapy, counseling, and/or social work strongly preferred.
  • Graduation from an accredited four-year college with a degree in Counseling, Criminal Justice, Social Work, Psychology, or other field related to the position is preferred.
  • Licensed Professional Counselor (LPC) is preferred.

Responsibilities

  • Works with Mental Health Counselors to provide assessment, counseling, re-integration, and/or rehabilitation services to program participants.
  • Assesses the needs, conditions, and desires of assigned program participants and provides case management services to program participants.
  • Conduct all VETcare Prevents services within the Zero Suicide framework.
  • Acts as advocate on behalf of program participants to improve access to vital services.
  • Participates in case conferences involving assigned program participants.
  • Completes/reviews all documents regarding clinical services and activities in accordance with licensing, Medicaid, and Joint Commission standards, and WestCare policies and procedures.
  • Conduct all VETcare Prevents services within the Zero Suicide framework as an as interdisciplinary team member.
  • Conduct community-based outreach to identify and engage program eligible individuals and their families.
  • Coordinate clinical services for emergency treatment for Veterans identified as at-risk of suicide.
  • With the Mental Health Counselor and in consultation with program participant, develop, monitor, and document individualized suicide prevention plans that include specific goals and objectives.
  • Provide referrals to participants and related activities (such as scheduling appointments) to help participants obtain needed suicide prevention services based on participants’ identified needs and goals.
  • Assist program participants in obtaining and coordinating the provision of VA benefits and other public health benefits and assistance with emergent needs including but not limited to health care services, daily living services, personal financial planning counseling services, temporary income support services, daily living services, personal financial planning counseling services, temporary income support services, employment searches, fiduciary and representative payee services, legal services, child childcare, transportation, and other allowable services.
  • Meets with program participants at least weekly to ensure resource linkages, follow-up, and provision of warm-hand offs to agencies and service providers to help participants obtain suicide prevention services.
  • Maintain daily contact with program participants through supportive calls, emails and/or text messages.
  • Must Embrace and embody the mission, vision, guiding principles, clinical vision, and goals of WestCare Foundation; and Perform all other duties as assigned.
