Service Coordination, Inc. (SCI) is seeking passionate Service Coordinators to work within the community and advocate for underserved individuals, particularly those with intellectual and/or developmental disabilities. SCI has been providing quality case management services since 1982, focusing on respecting the dignity and rights of individuals by connecting them to community resources. The role involves establishing positive working relationships, exploring individual interests and growth opportunities, and developing and monitoring annual Person-Centered Plans (PCPs) to ensure services meet state-mandated standards. Service Coordinators are expected to advocate on behalf of individuals, provide resource information, educate individuals and their families, and encourage self-determination and independence. The position requires collecting and documenting eligibility for DDA services, planning and updating PCPs, researching and coordinating resources, and monitoring these plans through on-site visits and follow-up activities. Adherence to SCI policies, HIPAA, and performance utilization benchmarks is crucial. The role also emphasizes collaboration, effective communication, customer focus, interpersonal skills, integrity, adaptability, and resourcefulness.
