The Multnomah County Health Department is seeking a compassionate advocate to join their team as the Case Manager Senior - RLRA Housing Retention. In this role, you will apply strengths-based practice to empower individuals as they transition to independent community living, ensuring that stable housing remains the foundation for their continued wellness and recovery. This highly independent role serves as the primary anchor for housing stability, specifically supporting Multnomah County Regional Long-Term Rent Assistance (RLRA) recipients who are transitioning out of or who have disengaged from Assertive Community Treatment (ACT) and intensive case management (ICM) levels of care. By applying trauma-informed de-escalation, rapid resource connection, and active landlord mediation, the Case Manager Senior resolves tenancy barriers, prevents evictions, and facilitates successful integration into independent community living. In all interactions, the position fosters a work culture of respect, trust, and understanding for the highly diverse populations served.
