About The Position

This full-time position involves providing comprehensive case management services to residents, focusing on their well-being, housing stability, and access to resources. The role requires direct engagement with residents, developing individualized service plans, assisting with benefit applications, and advocating on their behalf. The position also involves facilitating groups, organizing community-building activities, maintaining detailed documentation, and collaborating with internal and external stakeholders. The goal is to promote self-sufficiency, cooperation, and a sense of community among residents.

Requirements

  • Case Manager: High School Diploma + 4 years’ experience OR AA degree + 3 years’ experience OR CHW Certificate/CADC + 2-4 years’ experience OR BA/BS degree + 2-4 years’ experience OR MA/MS degree + 1 year experience.
  • Senior Case Manager: MA/MS + at least 2 years’ experience OR BA/BS + at least 4 years’ experience OR CHW Certificate/CADC + 4 years’ experience.
  • Knowledge of local social services and community resources for underserved populations and severely low-income individuals and families.
  • Patience, compassion, and empathy.
  • Excellent charting, recordkeeping, and professional writing skills.
  • Ability to quickly assess and respond to a variety of situations, including medical emergencies, psychiatric emergencies, conflict resolution, de-escalation, and other crises.
  • Computer skills and moderate experience using Microsoft Outlook, Word, and other MS Office applications.
  • Strong interpersonal skills, a team approach, and effective oral and written communication skills.
  • Demonstrated cultural competency in working with diverse, underserved populations.
  • Great collaborator with the ability to maintain professional, respectful, honest, and calm interactions.
  • Ability to work as part of a multi-disciplinary team.
  • Comfortable facilitating groups and hosting events and activities.
  • Commitment to the mission and values of Episcopal Community Services.
  • Fluent in both written and oral communication and comprehension in Spanish (for Bilingual positions).
  • Must secure finger image screening and annual TB screening.
  • Must be able to meet physical requirements of the position, including long periods at a desk/computer, some travel, and lifting up to 20 lbs.

Responsibilities

  • Perform intake assessment of assigned residents, including health status, psychosocial history, financial status, substance use history, and housing history.
  • Conduct regular outreach to residents and respond to incidents or documentation indicating a resident may be in crisis, need intervention, or be jeopardizing their housing.
  • Develop and implement individualized Service Plans based on resident goals and needs.
  • Provide referrals to internal or external resources for substance use, vocational training, legal services, immigration services, health services, housing, and money management.
  • Assess resident eligibility for benefits and entitlements and assist with applications and enrollment.
  • Assist residents with budgeting issues and develop voluntary money management plans.
  • Advocate on behalf of residents with Property Management regarding issues that may jeopardize their housing.
  • Work with Property Management to avoid eviction when possible.
  • Assist residents or staff with conflict resolution to ensure a comfortable living environment.
  • Participate in twice-per-week Wellbeing Checks and subsequent Emergency Safety Checks.
  • Develop and facilitate at least one weekly therapeutic/psycho-educational group.
  • Assist with the organization and execution of program events and community-building activities.
  • Maintain up-to-date, complete, confidential, and professional documentation in resident files.
  • Complete all necessary paperwork and administrative requirements within the required timeframe.
  • Assist residents with planned exits from the program, including comprehensive discharge planning and referrals.
  • Assist in developing and implementing new programs, groups, and activities.
  • Provide back-up coverage for other case managers.
  • Share information and new resources with residents and staff.
  • Follow and implement all applicable policies and procedures.
  • Establish and maintain positive working relationships with employees, volunteers, residents, and outside agencies.
  • Attend all assigned meetings.
  • Perform other duties as assigned.
