About The Position

The RN Case Manager serves as a leader of the complex discharge planning process and a valued member of the healthcare team. They collaborate with physicians and other members of the healthcare team to assess, plan, implement and evaluate proactive/complex/ safe discharge plans. Collaborates with MSW for guidance with psychosocially complex cases. The RN Case Manager is also responsible for facilitating coordination of care. The RN Case Manager along with the other members of the healthcare team accepts responsibility, authority, and accountability for management of care. This shall be in accordance with the current policies and procedures, mission, vision, and values of UVA Health Northern VA Culpeper. Serves as a resource for discharge planning and care coordination for medically complex cases. Maintains current knowledge of available community resources/post-acute care options.

Requirements

  • Associate's Degree in Nursing required.
  • Current RN licensure in state of employment required.
  • Weekend and holiday coverage required.
  • Constant standing, frequently walking, and bending/stooping.
  • Proficient communicative, auditory, and visual skills.
  • Attention to detail and ability to write legibly.
  • Ability to lift/push/pull 100 lbs. For any weight over 35 lbs., use Safe Handling Equipment.
  • May require the use of safety equipment, such as HEPA mask, for infection prevention.
  • On call responsibilities as directed.
  • Ability to travel between campus buildings, remote facilities, and out of town as needed.

Nice To Haves

  • 4 Year/Bachelor's Degree, BSN, preferred.
  • 2 or more years of healthcare experience preferred.
  • Case Management or related experience preferred.
  • Knowledge/Competence of Microsoft Office products and keyboarding.

Responsibilities

  • Serve as a leader of the complex discharge planning process.

  • Accept responsibility, authority, and accountability for management of care.
  • Serve as a resource for discharge planning and care coordination for medically complex cases.
  • Maintain current knowledge of available community resources/post-acute care options.
  • Interview and advise patients regarding health and illness prevention.
  • Synthesize data to determine preliminary diagnosis and therapeutic plan utilizing principles of prevention.
  • Recommend community resources to meet patient and family needs.
  • Collaborate with other departments or disciplines to achieve desired quality outcomes.
  • Monitor quality of core processes in department.
  • Apply the principles of safety and infection control to ensure the safety of all patients.
  • Provide primary health care and perform selective medical services under the direction of practice physicians.
  • Obtain complete history and physical data on patients.
  • Review patient records.
  • Interpret and integrate data to determine appropriate diagnostic and therapeutic procedures needed.
  • Perform a complete physical exam and record findings.
  • Establish presumptive diagnosis.
  • Order appropriate laboratory and diagnostic procedures.
  • Perform independent evaluations and treatment procedures in emergency situations.
  • Provide monitoring and continuity of care between physician visits.
