About The Position
The RN Case Manager assesses, plans, implements and monitors a comprehensive plan of care for her/his group of patients. This is done in collaboration with other members of the medical team, i.e. physicians, social workers, PT/OT staff, pharmacists, respiratory, dietary, and nursing.
Requirements
- Licensed RN in Florida.
- 2 years recent RN experience (in a clinical capacity) applicable to the designated population served is required.
- Valid RN license in the State of Florida.
Nice To Haves
- BSN preferred.
- Prior Case Management experience is preferred.
- Professional nursing board certification in specialty area preferred.
Responsibilities
- Maintain a focus on the patients' progression of care throughout the hospitalization, working with the care team on the appropriate utilization of resources/services and creating a safe, effective and efficient transition/discharge from the hospital.
