Requirements

• Current Licensure per state guidelines in clinical or related discipline OR a Bachelor's or Master's in a health or human services discipline.

Nice To Haves

• Previous experience in Case Management and Discharge Planning.
• CCM Certification.
• ADN or BSN Licensure.

Responsibilities

• Coordinate health care decisions by using a systematic approach to assure treatment plans that improve quality and outcomes.
• Coordinate care across the continuum.
• Promote cost-effective care within the allotted time frame.
• Assure payments of hospital-based services meeting patient-related utilization management criteria.
• Implement safe and appropriate discharge plans.
• Assess the psychosocial needs of the patient and provide intervention as part of the discharge planning process.
• Perform Clinical Interventions/Discharge Planning.
• Perform Care Planning Management.
• Perform Fiscal Management.
• Perform Payer/Referral Management.
