About The Position
Vasona Creek Healthcare is seeking a dedicated and compassionate Case Manager RN to join our full-time care team. This role is ideal for a licensed vocational nurse who excels in care coordination, interdisciplinary collaboration, and patient advocacy within a skilled nursing or healthcare setting.
Requirements
- Current RN license in good standing
- Prior case management, skilled nursing, or long-term care experience preferred
- Strong organizational, communication, and documentation skills
- Ability to work collaboratively in a fast-paced healthcare environment
- Proficiency with electronic medical records (EMR) systems preferred
Responsibilities
- Coordinate and manage patient care plans from admission through discharge
- Collaborate with physicians, RNs, therapists, social services, and interdisciplinary teams
- Monitor patient progress, treatments, and outcomes to ensure quality care
- Communicate effectively with patients, families, and healthcare providers
- Ensure compliance with facility policies, regulatory requirements, and documentation standards
- Assist with utilization review and discharge planning as needed
