About The Position
The Home Health RN Case Manager plans, coordinates, and manages the care provided to assigned patients. This position acts as the leader in collaborating with members of the interdisciplinary team, providing the foundation for creating the interdisciplinary plan of care. The Case Manager coordinates the various care processes that are necessary to achieve desired outcomes, and delegates, as appropriate. The Case Manager works to promote and sustain a culture reflective of Centra's best practices for quality and service, creates and maintains a safe patient environment, and contributes to a healthy work environment. The practice of the RN reflects the American Nurses Association's (ANA) Code of Ethics for Nurses, the Nurse Practice Act of Virginia, and the ANA Principles of Nurse Staffing.
Requirements
- Registered Nurse (RN) license in Virginia.
- Current Basic Life Support (BLS) certification.
- Proficiency in nursing case management principles and practices.
- Knowledge of the American Nurses Association's (ANA) Code of Ethics for Nurses.
- Understanding of the Nurse Practice Act of Virginia.
- Familiarity with the ANA Principles of Nurse Staffing.
Nice To Haves
- Experience in home health nursing.
- Experience in a case management role.
- Advanced Cardiac Life Support (ACLS) certification.
